MEDIC2 SUPPORTED LIVING CARE SERVICE

Introduction

Medic2 provides supported living services for Clients with learning disability, physical disability, young adults, elderly adults, and Clients with mental health.

Clients who live in their own home within the supported living scheme can choose our services from a few hours a week, or 24/7. These services cover support, personal care and life skills.

Anyone using our supported living service in any supported living scheme can expect to be treated with respect as a tenant in their own home.

Our service ensures that Clients feel safe and secure in their own home and within the community, so that they can enjoy an independent active life.

Aims & Objectives

Our overall aim is to enable our Clients become as independent as possible, to achieve their set outcome identified on the support plan.

Medic2 operates in the principles of care, in accordance with the guidance ‘valuing people principles’; privacy, dignity, independence, security, rights, choice, fulfilment, outcome and diversity.

We also adhere to the principles of Personalisation and believe that the client should be at the centre of their care. We follow a person centred approach to care and support; empowering the client to take the lead role and participate as fully as possible in all decision making processes.
Working with Clients

We support and encourage our Clients to do as much as possible for themselves and what they can’t, we will support all the way through. Therefore, our Clients will have the opportunity to learn and practice independent living skills.

In addition, our Clients are supported to become part  of the community, So if they choose to access any community facilities such as educational learning, local gym, swimming club, cooking club, bowling club, disco & parties and many more. The list available is exhaustive and the activities are dependent upon your choice, however subject to a risk assessment.

Our Support workers team are experienced and well qualified to maintain high standards of care, friendship and professionalism.

The Support Workers main role is to promote and enable each Client to live as independently as possible in their (own) homes and in the community as agreed within Clients' Support Plan. Staffs support Clients in all areas from life skills and social enablement to personal care support.

All staff has undergone intensive training programmes to ensure that a high quality of care is consistently delivered to achieve outcome.

We provide 24 hour staffing support on site: sleeping or waking night to ensure all the support needs of our Clients are catered for at all times.

Assessment

We work with Clients, family / relatives, advocates and care manager to build a support plan tailored to client’s needs and choice. Our support workers are trained and experienced in delivering a 'person-centred' approach. The support we provide is led by you ‘The Client’ and anybody else you choose to be involved

All new residents have their needs assessed before they move in to assess the level of support they need. We regularly review this so we can change it if a person's needs change. This is a detailed assessment to establish Client’s likes / dislikes, goals and support needed. In doing this our aim is to be able to understand our Clients needs and aspirations to enable them become as independent as possible.

Community Inclusion

We work in partnership with voluntary organization, advocacy groups, educational provider, care specialist and health advisory group to provide our Clients with a very variety of choice of service for a robust package

Health Care

We can support our Clients with health care if you need us to. Local health care will be primarily accessed unless any other needs have been identified.

Specialist professionals can be sorted on your behalf include opticians, podiatrists, psychologists, dietician, speech and language therapist, audiologist, loss & bereavement counselors etc
